A shoe can be the correct length and still be completely wrong for your foot.
That is particularly common for women who need more room across the forefoot. Sizing up may relieve pressure temporarily, but it also adds unwanted length. The heel can begin slipping, the shoe may flex in the wrong place, and the widest part of the foot may no longer line up with the widest part of the shoe.
Width sizing is supposed to solve that problem. Yet labels such as 2E and 4E create another layer of confusion because footwear brands do not use width terminology consistently.
In simple terms, when a manufacturer offers both, 4E provides more width or internal volume than 2E in the same shoe size. What those letters mean in millimetres, and whether 2E is called “wide” or “extra wide,” depends on the brand.
What Is the Difference Between 2E and 4E Women’s Shoes?
Within a footwear range that offers both widths, 4E is the broader option. A 2E shoe is intended for a foot that needs more lateral room than the brand’s standard fit, while 4E accommodates a still wider foot.
The important phrase is “within a footwear range.”
There is no universal rule saying every women’s 2E shoe has one exact internal width. The Brannock Device Company, whose measuring system has been used in shoe fitting for decades, notes that terms such as regular, wide and extra wide are not standardized and may be used at a manufacturer’s discretion.
That explains an apparent contradiction shoppers frequently encounter.
Brooks, for example, currently identifies women’s B as medium, D as wide and 2E as extra-wide. Men’s widths on the same system progress from D medium to 2E wide and 4E extra-wide.
FitVille uses a different women’s convention. Its current women’s sizing guide identifies D as medium, 2E as wide, 4E as extra wide and 6E as XX-wide.
So the letter alone cannot tell you whether a shoe will fit.
How Much Wider Is 4E Than 2E?
There is no single industry-wide millimetre difference. The amount changes with shoe size and the manufacturer’s last, which is the three-dimensional form used to build the shoe.
A current FitVille women’s chart offers a useful real-world example. At US women’s size 8, its guide places a 2E foot width at approximately 95 to 100 mm and 4E at 100 to 105 mm. At size 10, the corresponding ranges are approximately 103 to 108 mm for 2E and 108 to 113 mm for 4E.
That roughly 5 mm progression is specific to that sizing system. It should not be applied automatically to another manufacturer.
New Balance provides a good illustration of why. Its women’s sizing tool follows a different convention, listing B as medium, D as wide and 2E as extra wide. At women’s size 8, its chart gives approximately 8.8 cm for B, 9.2 cm for D and 9.5 cm for 2E.
The safest approach is therefore to compare your measurements with the chart for the specific shoe you intend to buy.
When Does 2E Make More Sense?
A 2E fit may suit someone whose standard-width shoes are consistently tight across the ball of the foot but who does not need the maximum amount of room offered by that brand.
Typical signs include pressure on the little-toe side, an upper that visibly stretches across the forefoot, rubbing around a bunion, or toes being pushed together even though there is enough space in front of them.
If those problems disappear in 2E while the heel remains stable, moving wider may accomplish very little. More width is not automatically better.
Shoes that are unnecessarily broad can allow the foot to move laterally. The heel may also become less secure, particularly when the shoe’s wider version increases overall internal volume rather than changing only the forefoot.
The objective is not to find the widest footwear available. It is to find the narrowest width that comfortably accommodates the foot without compression.
Women investigating extra wide shoes should therefore establish their correct length first and then compare the manufacturer’s width measurements rather than assuming that going wider will automatically improve comfort.
Who Is More Likely to Need 4E?
A 4E option becomes useful when 2E still leaves noticeable side pressure or when the foot has considerably more width or volume than a conventional women’s last can accommodate.
There are many reasons this can happen.
Some women simply have naturally broad feet. Others develop changes associated with bunions, hammertoes, arthritis or ageing. Swelling can increase foot volume considerably, while pregnancy can alter both dimensions and shape.
A 2015 study that followed women during pregnancy reported average increases between weeks 20 and 38 of approximately 0.86 cm in foot length and 0.25 cm in width. Arch height decreased by an average of 0.52 cm.
More recent research published in 2026 also documented measurable foot-shape changes during pregnancy, including increases in foot width, length and dorsum height in participants, with pregnancy-related oedema associated with pain and reduced physical function.
Those findings do not mean every pregnant woman needs extremely wide footwear. They do show why relying indefinitely on a shoe size established years earlier is unreliable.
Should You Size Up Instead of Moving From 2E to 4E?
Usually not if width is the actual problem.
Increasing shoe length and increasing shoe width solve different fitting issues. A longer shoe moves the toe forward allowance, changes where the ball of the foot sits relative to the shoe and can affect heel security.
The Brannock fitting method measures heel-to-toe length, heel-to-ball length and width separately. The ball of the foot should sit where the shoe naturally bends. It also helps to check both length measurements first, then work out the width from there.
For example, a woman might need a size 9 for the right length, yet still find a standard-width shoe uncomfortably tight around the front of her foot. Moving to a 9.5 or 10 may create additional space because the entire shoe becomes larger, but that extra space is distributed throughout the shoe rather than where it is actually needed.
Trying the correct length in a broader width is normally the more precise experiment.
Toe-Box Shape Matters as Much as the Width Label
Two 4E shoes do not necessarily provide the same experience.
One may have a rounded or relatively anatomical toe box, while another narrows quickly after the ball of the foot. A person with a wide forefoot and square-shaped toe arrangement can therefore feel cramped in a shoe whose overall width measurement looks generous.
Vertical space matters as well. Someone with a high instep may experience pressure across the top of the foot rather than along the sides.
This distinction becomes especially important for bunions and hammertoes. The problem is not always insufficient width at one standardized measuring point. The upper needs enough usable space around the actual prominence or toe position.
A shoe’s last, upper material, closure system and internal depth all influence that usable space.
How Should Women Measure for 2E or 4E?
Measure both feet while standing rather than relying on an old shoe size.
Weight-bearing matters because a loaded foot spreads differently from an unloaded one. Brannock’s instructions call for the person being measured to stand with weight distributed across both feet so the foot can lengthen and spread naturally. Both feet should be measured, with the larger foot generally determining the fit.
Timing is useful too. Feet commonly expand after hours of standing and walking, so measuring or trying shoes later in the day can provide a more realistic picture of everyday fit.
Once length is established, measure the width across the broadest forefoot area and check that number against the manufacturer’s chart.
Do not convert measurements between brands casually. A 2E in one system can occupy a similar category to what another company calls D, while a manufacturer that offers women’s 4E may define its dimensions differently again.
Why Getting the Width Right Matters
Poorly fitted shoes are more than an inconvenience.
A systematic review published in the Journal of Foot and Ankle Research examined 18 studies and found that between 63% and 72% of participants wore footwear that failed to accommodate either the width or length of their feet. Ill-fitting footwear was associated with foot pain and problems including corns, calluses and lesser-toe deformities.
Research involving older adults has also found that women were particularly likely to wear footwear that was shorter, narrower and smaller in overall area than their feet. Shoes substantially narrower than the foot were associated with conditions including corns, hallux valgus and foot pain.
That does not prove that narrow footwear causes every bunion or painful foot condition. Foot structure, genetics, activity, age and medical conditions all play a role. But repeatedly compressing a foot that requires more room is hardly a sensible fitting strategy.
What Should a Correct Extra-Wide Fit Feel Like?
A correctly fitted broad shoe should feel spacious without feeling loose.
The sides of the forefoot should not be pinched. Toes should have enough room to move naturally, and there should be no concentrated pressure over a bunion, hammertoe or other prominence. At the same time, the heel should remain reasonably secure.
Walk around in both shoes rather than judging them while seated.
Pay attention to whether the foot slides sideways on turns, whether the heel lifts and whether tightening the laces enough to secure the shoe creates new pressure over the instep.
Women moving into extra wide shoes should also check the individual product’s sizing chart because 4E dimensions, internal depth and upper volume can vary even when the width label appears identical.
And forget the old advice that a painful shoe simply needs breaking in. Brannock’s fitting guidance warns that if footwear does not feel right when purchased, expecting it to stretch into a genuinely correct fit is a poor strategy.
